DOG APPOINTMENT

For an appointment at Collier Spay Neuter Clinic, you must complete ALL fields.

If you have more than 1 pet, you will need to fill out a separate form for each pet.
We will make every effort to schedule dates together for multiple-animal households.

Your appointment will be confirmed by a follow up email within two business days, so be sure to check your SPAM filter if you have not heard from us within that timeframe.

  5. Drop-off is at 8:00 a.m regardless of the day and/or time of your pet's surgery. We do not do surgeries on Fridays. If you need special accommodations, please let us know prior to the date of your appointment and we will do our best to assist you.

  12. If your dog is possibly pregnant, cryptorchid (undescended testicles), or anything else we need to be aware of, please inform us below.








  15. Rabies vaccinations must be current for all pets needing surgery.
